Please complete this form with reference to photograph usage throughout the school year.
Universal Trip Permission Slip
This form will encompass permission for all class trips, athletic events, and other activities.
However, you will be notified by way of the Monthly Calendar and classroom teachers or coaches
in advance about specific details of all trips and activities during the school year.
Bus - Pennsylvania Act 372
Only sign this form if you are requesting and will be definitely using a school bus.
If you used the bus last year, but your situation has changed, please let the front office know.
Technology Agreement
All students in St. Edmund’s Academy in grades 4 through 8 must read and sign the agreement regarding
the appropriate use of the new technologies. Parents, too, must read and sign this agreement.
Dental Forms (students entering Grades K, 3, and 7 only)
Extended Day Program Form
St. Edmund’s Extended Day Program will be available to all students in PreKindergarten through
fourth grade during the school year. This program is held from 3:30 P.M. - 6:00 P.M. each school day.
Birthday Book
A book of a student’s choosing may be purchased and given to the St. Edmund's Library
on the occasion of his/her birthday.
